Instructions for completing survey:

  • Write in the transaction number (also shown on your receipt)
  • Input the receipt month, day and year in each dropdown field
  • Select your country (USA or Canada)
  • After entering all the required information, select the “next” arrow icon.
  • You will now be asked some straightforward questions about your previous shopping experience with Banana Republic. Try to answer the questions as honestly you can.
  • During the course of the survey you will also be required to provide your email address. Make sure this is your correct email address in order to receive the discount.
